3. For any type of Gift Certificate, please include the recipient's Name and Address in the “Shipping Address” fields on the Checkout page.

4. If you’ve chosen to receive a Gift Certificate via E-Mail and wish for it to be delivered to an email other than your own, include the recipient’s e-mail address in the “Comments” Field prior to submitting your order.

5. Include any personal messages for Gift Card in the “Comments” Field prior to submitting your order.
